A population pharmacokinetic model for the CTLA-4 modulator gotistobart, developed from 632 patients across three phase I-III trials, defines its dosing regimen and identifies covariates with limited clinical impact on exposure.

This study aimed to develop a population pharmacokinetic (PK) model for gotistobart, a next-generation humanized IgG1 monoclonal antibody targeting CTLA-4, to characterize its pharmacokinetics and identify covariates contributing to inter-individual variability in patients with advanced solid tumors. Gotistobart is engineered to preserve CTLA-4 through endosomal recycling while enhancing intratumoral regulatory T-cell depletion. The model was developed to support exposure-response analyses and regimen selection for this investigational agent.

The population PK model was developed using serum concentration-time data obtained from 632 patients with advanced solid tumors enrolled in three phase I-III clinical trials. In total, 3,406 pharmacokinetic observations were collected across a dose range of 0.1 to 10 mg/kg. A two-compartment model with linear elimination best described the data. Inter-individual variability was included on clearance, central volume, and peripheral volume. Parameter estimates were precise, with typical values of 0.005 L/h for clearance, 0.021 L/h for inter-compartmental clearance, 2.59 L for central volume, and 3.43 L for peripheral volume. Covariate analysis evaluated the impact of body weight, albumin, and sex on PK parameters.

The final population PK model successfully characterized gotistobart's disposition. A two-compartment model with linear elimination provided the best fit. Typical parameter estimates were: clearance of 0.005 L/h, inter-compartmental clearance of 0.021 L/h, central volume of 2.59 L, and peripheral volume of 3.43 L. Covariates identified in the model included body weight and albumin on clearance and volumes, and sex on volumes. The analysis revealed that these covariates had limited clinically relevant impact on gotistobart pharmacokinetics. Goodness-of-fit plots confirmed the model's ability to describe the data adequately.

The developed population PK model provides a quantitative foundation for subsequent exposure-response analyses and regimen selection. Simulation results indicated that administration of two 10 mg/kg loading doses followed by 6 mg/kg every 3 weeks accelerated the attainment of steady state compared with 6 mg/kg every 3 weeks alone. This finding has implications for dosing strategies designed to rapidly achieve target exposures. Furthermore, the identification of covariates with limited clinical impact suggests that dose adjustments based on these specific patient factors may not be necessary for the majority of patients.

# Klinik önem

The validated population pharmacokinetic model serves as a critical tool for optimizing gotistobart dosing regimens. By defining typical PK parameters and assessing the impact of covariates, the model supports the design of future clinical studies and the interpretation of exposure-response relationships. The finding that a loading dose regimen accelerates steady-state attainment offers a practical strategy for achieving pharmacological effect more quickly in a clinical setting. However, the limited impact of identified covariates indicates that routine dose adjustments based on body weight, albumin, or sex may not be required based solely on PK considerations.

Several limitations should be considered when interpreting these results. The population PK model was based on data from phase I-III trials; its generalizability to later-phase or real-world settings has not been established. The covariates identified in the model explained limited inter-individual variability, suggesting that other factors may contribute to PK differences. Model validation relied on parameter precision and goodness-of-fit; external validation data were not reported. Additionally, the phase I-III trials may not represent all advanced solid tumor populations, and no direct comparison of clinical outcomes linked to PK parameters was reported in the provided data.
